USCIS Immigration Medical Exams (Form I-693)

At Fishburn Medical, we provide complete, efficient, and compassionate USCIS Immigration Medical Examinations for individuals and families completing their Form I-693. Our experienced clinical team includes licensed physicians, trained medical assistants, and administrative staff who work together to make the process smooth, accurate, and stress-free.

Your immigration medical evaluation is completed in two steps:

  1. The Exam Appointment
    During your initial visit, our clinical staff performs your physical examination, reviews your medical and vaccination history, collects any required labs, and determines which vaccines you may need for USCIS compliance. This is the main visit where all testing and documentation requirements are identified and completed.
  2. Packet Pickup Appointment
    After all laboratory results are finalized and any required vaccines have been completed, we prepare your official paperwork. You will then return for a brief second appointment to pick up your fully completed I-693 packet. At pickup, you will receive:
    • A sealed envelope for USCIS
    • An unsealed copy for your records

Laboratory Testing and Vaccinations

USCIS requires specific laboratory tests depending on age and medical history. These may include:

  • •Tuberculosis testing (IGRA blood test preferred)
  • •Syphilis screening
  • •Gonorrhea screening
  • •Additional tests if medically indicated

Fishburn Medical offers two options for required labs:

  1. Cash-Pay On-Site Labs – Convenient, competitive pricing completed at our clinic.
  2. External Laboratory Option – We can order your labs, and you may complete them at an outside facility of your choice.

We do not accept insurance of any kind for USCIS-related services, including laboratory testing.


Vaccinations
USCIS requires applicants to be up-to-date on specific vaccinations. At Fishburn Medical, we carry many of the commonly required vaccines. Please call for further information.

What to Bring

Please bring the following items to your appointment:

  • A government-issued photo ID (passport, driver’s license, or state ID)
  • Any vaccination records, even if incomplete or in another language
  • Any prior TB testing documents
  • Your immigration attorney’s information (if applicable)

For minors, a parent or legal guardian must be present.

Pricing and Payment Information

We provide transparent, straightforward pricing for all USCIS services. The base USCIS exam fee includes:

  • Civil Surgeon–supervised medical examination
  • Comprehensive record review
  • Completion and sealing of Form I-693
  • Unsealed copy for your records

Required laboratory tests and vaccinations are not included in the exam fee and are priced separately.

Patients may choose one of the following for required labs:

Vaccinations, when needed, are also billed separately.

Fishburn Medical is a self-pay clinic for USCIS services. We do not accept insurance for exams, labs, or vaccinations. Payment is due at the time of service.
