Helping Commercial Drivers Stay Safe and Certified

At Fishburn Medical, we provide Department of Transportation (DOT) physicals for commercial motor vehicle (CMV) drivers, as required by the Federal Motor Carrier Safety Administration (FMCSA). Our certified medical examiners ensure you meet the health and safety standards to operate commercial vehicles confidently and legally.

What is a DOT Exam?

A DOT physical is a federally mandated medical examination required for individuals who operate commercial vehicles. The exam determines whether a driver is physically and mentally fit to safely perform safety-sensitive duties, such as long-haul trucking or bus driving.

We conduct exams in accordance with FMCSA guidelines and issue the Medical Examiner’s Certificate (Form MCSA-5876) when all requirements are met.

Who Needs a DOT Physical?

You need a DOT exam if you:

  • Operate a vehicle with a gross vehicle weight rating (GVWR) of 10,001 lbs or more
  • Transport hazardous materials requiring a placard
  • Operate vehicles designed to carry more than 8 passengers (for compensation) or more than 15 passengers (not for compensation)
  • Are subject to FMCSA regulations for interstate or intrastate commerce

What to Expect at Your DOT Exam

Your DOT physical includes:

  • Medical history review
  • Vision and hearing screening
  • Blood pressure and pulse check
  • Urinalysis (checks for underlying medical conditions, not drugs)
  • Physical examination (neurologic, musculoskeletal, cardiovascular, etc.)

If you meet all medical standards, we’ll issue your Medical Examiner’s Certificate, valid for up to two years (shorter intervals may apply for certain conditions).

What to Bring

  • A valid government-issued photo ID
  • A list of current medications and doses
  • Glasses or hearing aids, if needed
  • Medical records or clearance letters for any chronic conditions (e.g., diabetes, sleep apnea, cardiac issues)
  • Most recent Medical Examiner’s Certificate (if renewing)

We make every effort to keep your exam efficient and informative.

Common Conditions Requiring Additional Documentation

  • High blood pressure
  • Diabetes (especially insulin use)
  • Sleep apnea (CPAP compliance reports)
  • Heart disease
  • Neurologic disorders (seizures, stroke, etc.)
  • Psychiatric or substance use history

If you’re unsure what you need, call us before your appointment—we’ll guide you through it.
