What is a SAP Evaluation?
Under 49 CFR Part 40, a DOT SAP evaluation is required for safety-sensitive employees who:
- Tested positive on a DOT drug or alcohol test
- Refused to test
- Engaged in prohibited drug/alcohol-related conduct
As a qualified SAP, we assess your situation, recommend appropriate education or treatment, and oversee your successful completion of the return-to-duty process.
This is a federally mandated process that must be followed exactly—only a certified SAP can perform these evaluations.
Who Needs a DOT SAP Evaluation?
DOT-regulated employees in the following industries:
- Federal Motor Carrier Safety Administration (FMCSA – truck/bus drivers)
- Federal Aviation Administration (FAA – pilots, flight attendants)
- Federal Transit Administration (FTA)
- Federal Railroad Administration (FRA)
- Pipeline and Hazardous Materials Safety Administration (PHMSA)
- United States Coast Guard (USCG)
If your employer or TPA referred you for a SAP evaluation, you are not eligible to return to work in a safety-sensitive role until the SAP process is complete.
The SAP Process at Fishburn Medical
- Initial Evaluation: We assess your violation, medical history, and any previous treatment. We recommend a personalized education or treatment program that meets DOT criteria.
- Referral and Monitoring: You complete the required education or treatment with an approved provider. We remain in contact with your provider and track your progress.
- Follow-Up Evaluation: Once your program is complete, we meet again to determine if you're eligible for return-to-duty testing. If you’ve successfully complied, we issue written documentation to your employer or TPA.
- Return-to-Duty Test: Your employer arranges a DOT return-to-duty test. You cannot return to safety-sensitive duties until this test is negative.
- Follow-Up Testing Plan: We create a required follow-up testing schedule (minimum 6 tests in 12 months, up to 5 years).
